Other additions to the program include Michigan’s Affinia Health Network, Arizona’s Banner Health, Colorado’s Boulder Valley Care Network, Massachusett’s Lahey Health, Michigan’s MedNet One Health Solutions and Wexford Physician Hospital Organization, California’s Sharp HealthCare, Florida’s Specialized On-Site Care Management and Germany’s Rostock University Medical Center.
Philips Wellcentive platform, which it purchased last July, provides a cloud-based informatics platform that aggregates and analyzes clinical claims and financial data with the goal of improving quality and revenue growth, the company said.
“For the future success of HealthOne, we recognized the need for a comprehensive solution to enable more effective care management Philips has significant experience in supporting organizations as they transition to value-based care and managing risk-based contracts. As we work to drive a healthier bottom line, Philips Wellcentive supports our growth strategy and helps us identify gaps in our care delivery and improve the use of our data,” HealthOne chief information officer Andy Draper said in a press release.
Rostock University Medical Center will integrate both the Wellcentive analytics platform and Philips eCareCoordinator telehealth technology to support cardiac care management, the company said, which allows patients with heart disease to monitor vitals from home and clinicians to view patient data to monitor health and progress.

As health systems move beyond care delivery to health delivery that starts and ends in the home, ensuring they have the right data with actionable insights will be critical,” Philips population health management biz lead Derek Ross said in a prepared statement.
Earlier this month, Philips said it won FDA 510(k) clearance for its eL18-4 ultrasound transducer designed for ‘small parts’ assessments to detect abnormalities in small organs close to the skin.
